The Four Rotator Cuff Muscles

The rotator cuff is formed by four muscles that originate on the scapula (shoulder blade) and insert through a shared tendinous cuff onto the greater and lesser tuberosities of the humerus (upper arm bone). Each muscle has a distinct role in shoulder movement and joint stability, and each is susceptible to tearing at different locations and under different loading patterns.

The supraspinatus runs from the supraspinatus fossa of the scapula, through the subacromial space, to the greater tuberosity. It initiates abduction (raising the arm to the side) and is by far the most commonly torn rotator cuff muscle, with tears typically occurring at the "critical zone" approximately 1 cm from the insertion, where blood supply is relatively poor. The subacromial space narrows with arm elevation, and the supraspinatus tendon is repeatedly compressed between the humeral head and the acromion in this position, which is the mechanism for impingement-related supraspinatus degeneration and tearing.

The infraspinatus and teres minor are the primary external rotators of the shoulder and insert posteriorly on the greater tuberosity. The infraspinatus is the second most commonly torn cuff muscle and is a major source of posterior and lateral shoulder pain through trigger points even when no structural tear is present. The subscapularis, the largest of the four rotator cuff muscles, is the primary internal rotator and is less commonly torn, but when subscapularis tears occur they significantly impair internal rotation strength and are often associated with anterior shoulder pain and instability.

MRI findings should always be interpreted alongside clinical examination. A 2015 systematic review found that up to 34% of asymptomatic individuals have rotator cuff tears visible on MRI. If your MRI shows a rotator cuff tear but your symptoms do not match the expected pattern for that tear, the imaging finding may not be the primary driver of your pain, and treatment targeting it specifically may miss what is actually causing your symptoms.

The Role of Trigger Points in Rotator Cuff Pain

One of the most clinically significant and underappreciated aspects of rotator cuff pain is the role of myofascial trigger points in the rotator cuff muscles and surrounding musculature. Active trigger points in the infraspinatus reproduce lateral shoulder and upper arm pain that is indistinguishable from supraspinatus tendon pain to most patients. Subscapularis trigger points refer to the posterior shoulder and can produce severe limitation of external rotation that mimics adhesive capsulitis. Supraspinatus trigger points produce pain directly over the lateral shoulder and may create impingement-type symptoms by altering the resting position of the humeral head in the glenoid.

Many patients who have been told their shoulder pain is from a rotator cuff tear find that systematic treatment of myofascial trigger points produces dramatic improvement without any structural change in the tear. This does not mean the tear is irrelevant, but it indicates that the trigger points, rather than the torn tissue itself, are often the primary pain generators. The decision between conservative and surgical management for rotator cuff tears is one of the most actively studied questions in shoulder surgery, and the evidence is considerably more favorable to conservative management than many patients are told. A randomized controlled trial by Moosmayer et al. (2019), with 10-year follow-up, compared surgery versus exercise therapy for small to medium full thickness supraspinatus tears and found no significant difference in outcomes at 10 years. Patients who initially responded poorly to exercise and crossed over to surgery achieved similar final outcomes to those who had surgery earlier, suggesting that watchful waiting with conservative management does not compromise eventual outcomes for patients who later require surgery.

Current clinical guidelines from the American Academy of Orthopaedic Surgeons support a minimum 6-week trial of conservative management, including targeted exercise rehabilitation and pain management, before surgical consultation for degenerative full thickness tears. Many guidelines recommend 3-6 months of conservative care. The exceptions, where earlier surgical discussion is warranted, are acute traumatic full thickness tears in younger, active patients, where the risk of tendon retraction and fatty infiltration of the muscle belly increases the longer surgery is deferred.
